At Lifetime Eyecare the health and well-being of our patients, employees, and our community is our top priority. We understand the concern and uncertainty you may be experiencing surrounding the coronavirus (COVID-19) and we are committed to being responsive to the needs of our patients as the situation evolves. Our office remains closed to the public, but open on an appointment-only basis.
In our effort to keep everyone safe and healthy we have adjusted our office procedures. Please take the time to read all of the steps our office is taking to keep you and your family safe.
-Every person that enters the office must wear a face mask.
-All patients that are new to our office or have not been to our office in the past two years must fill out our Patient Registration forms online before your appointment. https://www.lifetimeeyes.com/new-patient-…/web-registration/
-The office doors will stay locked. When you arrive at the office for your appointment, please stay in your car and call us to let us know you are there. We will come to the door to let you in.
-All patients will need to do the Optos Photography in lieu of dilation at this time. Doing this will decrease the amount of physical contact you will have with our staff. There is an additional $30 charge for the Optos Photography.
-We ask that all patients come to the office alone. Any patient under the age of 18 may be accompanied by one adult.
-Any patient that has had any symptoms or been around anyone with symptoms such as cough, shortness of breath, sore throat, fever, chills, muscle pain, headache, or new loss of taste or smell will need to reschedule their appointment.
-Any patient that has been exposed to anyone with COVID-19 in the past 14 days will need to reschedule.
